Why Should I Participate in the Tivity Health Fitness Reimbursement Program?
The Tivity Health Fitness Reimbursement Program will bring new members to your facility. You will be marketed by Tivity Health and you will receive your specified monthly rate from the member. There is no cost to you to participate in this network.
The Tivity Health Fitness Reimbursement Program is a fitness benefit made available to members of Tivity Health clients. Eligible members select and pay for a membership at the participating location of their choice. The member may be eligible for reimbursement from their health plan or employer. The rules and schedule for member reimbursement vary per Tivity Health client.

What Do I Do Next?
Simply complete the registration form by clicking the Register button below. Please have the following details available:
- Your business information – name, type (LLC , Partnership), and Federal Tax Identification Number
- Pricing - While as a participating PL in our reimbursement network you are free to set your pricing at whatever level you desire, we encourage you to be as competitive as possible in enticing as many new member sign-ups from our prospects
